All nurses at Care@Home are certified and shine in their relevant fields. They work on assisting the patient to the best of their abilities, handling every case in a singular manner and making sure that each individual receives the care that is best adapted to his particular condition.
Our registered nurses provide skilled medical care for a variety of conditions:
Medication Management: Partnering with the patient’s doctor, our nurses develop a medication schedule, evaluate the effectiveness of medications and educate the patient and/or his or her caregiver on proper dosage, frequency and side effect.
Home infusion/IV therapies: Our nurses manage IV medications and nutritional support, like tube feeding at home, and they teach the caregivers how to manage nutritional support
Disease Management: For patients with chronic conditions, such as heart failure, diabetes, and chronic obstructive pulmonary disease (COPD), our Disease Management programs aim to help patients maintain independence and a healthier lifestyle. Services include nursing care, rehabilitation, patient education, and in-home patient monitoring.
Wound care: For patients with complex, non –healing wounds or new stage 3 and 4 pressure ulcers, our certified wound care nurses provide specialized, in-home care and education to ensure wounds heal.
Ostomy care: With the guidance of our certified wound, ostomy and continence care nurses, patients learn how to care for an ostomy and live comfortably with an ostomy pouch.
Palliative care: The goals are to lessen a patient’s pain, control their symptoms reduce hospitalizations and ease their mind
Diabetes management: Our nurse will evaluate your situation and propose a personal care plan to meet your specific needs: Home safety, Safe medication administration, Good quality of life with the disease, Ability to control the diabetes
Practical nurse: To assist patients in resuming their daily routine at home, we provide home health aides to help with activities of daily living. These activities include bathing, grooming, dressing, walking and much more
